Household Tasks
Companion trained and compassionate support workers assist our participants with general cleaning and maintenance activities that they are unable to do so themselves without assistance. Some of the tasks we assist our clients with include; de-cluttering, house cleaning, organising, and other simple domestic duties that promote participant safety and comfort in their homes.

Daily Living
Companion Health will support you with Daily Living Assistance to enable you to live according to your choices. Companion provides flexible person-centred daily living support that meet your specific needs, goals and outcomes. The services are provided to you at a place of your choice including but not limited to home, in the community, at school or work. The services will be available to you 24/7 depending on your needs.

Community Participation
Companion Social and Community will assist you in the community and assist our participants to achieve desired goals including recreational activities. This is to promote participant well being and inclusion in the community.

Support Coordination
Companion offers all levels of support coordination including level 3 which provides participants with complex needs or high levels of risks. The specialist team manages complex barriers impacting your ability to implement your plan and access appropriate support. Our team will assist you to reduce complexity in your support environment and overcome barriers with a view to connecting you with broader systems of supports as well as funded supports.

Community Nursing
Companion's Community Health Nursing team assists participants and their families to manage various health conditions. The nurses aim to support and promote participant independence, safety, and a healthy lifestyle. In addition, the nurses provide personalised care specific to participant needs. Companion Nurses provide a Person-Centred approach while caring for our participants. This means that the participant decides the nursing support they require and how they are provided.

Companion provides specialist disability accommodation (SDA) meeting specific needs with the understanding that your home must be a place of comfort and relaxation to promote your well-being and safety. Our facilities foster your independence while we support you without barriers. The facilities are spacious, fully wheelchair accessible and a place to call home.

Assist - Life Stage Transition

Accommodation/Tenancy
Companion Health is one of the major providers of disability accommodation in WA. Fully registered under the NDIS therefore, we support people in our homes and theirs. We can assist you to identify appropriate housing and accommodation to suit your needs. Our aim is to provide guidance and support to our participants so they may look after their homes and meet their tenancy requirements.

Assist - Travel / Transport
Companion will arrange transport for you to attend your community access including all your appointments. We provide wheelchair accessible transport/vans which reduce your limitations.
